Strong communication technology skills will be essential as you will use Electronic Health Records (EHR) systems to maintain accurate documentation and client profiles.

Key Responsibilities\n

• Conduct thorough, ongoing psychosocial assessments for seniors, analyzing their emotional, mental, and social needs to design effective care interventions.

• Collaborate with clients and their families to formulate personalized care plans that account for individual preferences and life circumstances.
\n
• Provide empathetic counseling and emotional support to help seniors navigate transitional phases, such as health crises or losses, addressing their concerns and challenges.

• Advocate for clients, ensuring they have access to community resources like support groups, health services, and recreational activities tailored for seniors.

• Work hand-in-hand with an interdisciplinary team comprising healthcare professionals, caregivers, and nursing staff to ensure a holistic approach to senior care.

• Engage in regular remote check-ins and home visits (as necessary) to monitor the progress of clients, assess their living conditions and adjust care plans as needed.

• Facilitate family meetings to improve communication among relatives involved in the elderly client’s care and resolve any potential conflicts.

• Stay updated on current practices in social work and relevant policies, ethically managing client interactions and documentation.

Your primary focus will be on the elderly, providing them with tailored social services designed to address a variety of issues they face. These can include mental health concerns, social isolation, and difficulties in navigating healthcare systems. By advocating for their needs and linking them with appropriate resources, you will play an essential role in enhancing their overall quality of life.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• A valid and current license as a Licensed Master Social Worker (LMSW) in the state of Colorado.

• Minimum of two years of experience in a social work role, ideally within a community service or senior care environment.

• Strong knowledge of social work principles, particularly as they relate to issues faced by aging populations.

• Highly developed assessment and problem-solving skills.
