This position offers a remote work setting, allowing flexibility for the social worker to provide essential care and support that meets the needs of clients in a Hospice Center in Fort Collins, Colorado. Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, focusing on their emotional and social circumstances and recognizing the impact of aging.

• Provide ongoing counseling and emotional support, helping clients navigate complex life transitions such as impending loss and bereavement.

• Act as a client advocate to ensure their rights are protected and they have access to critical community resources, including financial and medical assistance.

• Travel for in-person client visits when necessary, conducting evaluations of clients' living environments and addressing safety concerns.

Qualifications\n
