Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their counseling needs, utilizing DSM criteria for substance abuse diagnosis.

• Develop and implement individualized treatment plans that align with ASAM criteria to facilitate appropriate care levels.
\n
• Document all aspects of counseling services, including assessments, treatment progress, referrals, and follow-ups, in each client's clinical record.
\n
• Maintain an organized caseload, ensuring case management and treatment planning duties are consistently completed, with regular utilization review updates.
\n
• Provide advocacy and coordination of support services for clients and their families, recording the services provided and engaging with relevant community resources.
\n
• Facilitate interventions during crisis situations through a collaborative approach with a multi-disciplinary team to support client stability and recovery.
\n
• Prepare discharge and continuum of care plans that guide clients in transitioning to appropriate levels of support post-treatment.

Patient Population / Client Focus\n

Your work will directly impact those struggling with mental health issues, particularly individuals coping with substance use disorders and other co-occurring conditions in a Hospice environment. This role requires a deep understanding of the sensitivities surrounding end-of-life care, promoting dignity and empowerment for clients and their families during vulnerable times.

Qualifications\n

• A Bachelor’s Degree in Social Work or a related field.

• Current license as a Clinical Social Worker (LCSW) in good standing.
