This full-time remote position is based in Fort Collins, Colorado, within our Hospice Center framework. The LCSW will provide clients suffering from severe emotional disorders and major mental illnesses with the support they need to achieve better mental health outcomes.

Key Responsibilities\n

• Provide direct clinical services to clients suffering from major mental illnesses and severe emotional disorders, both in the office and community settings.

• Conduct evidence-based, developmentally appropriate assessments and screenings to evaluate client needs.
\n
• Create and implement integrated care plans, linking clients with necessary services and facilitating communication among clients, families, and healthcare providers.
\n
• Perform individual, group, and family therapy sessions tailored to each client’s unique circumstances and treatment goals.
\n
• Participate actively as a team member, collaborating with colleagues to advance team goals and improve service outcomes.
\n
• Monitor and report unsafe situations within the workplace, ensuring that a safe and supportive environment is maintained for clients and staff.
\n
• Provide crisis intervention as needed, prioritizing immediate support for clients in distress.
\n
• Stay current with best practices in the field, ensuring timely completion of documentation and adherence to ethical guidelines.
\n
• Engage in quality improvement initiatives through committee participation and feedback, contributing towards the enhancement of our agency's service delivery.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• License as a Clinical Social Worker (LCSW) in Colorado.

• Strong verbal and written communication skills, with the ability to convey complex concepts in understandable terms.

• Expertise in assessment, diagnosis, and treatment planning for diverse populations, particularly those facing severe emotional and mental health challenges.

• Ability to work effectively within multi-disciplinary teams and demonstrate cultural competency in interacting with clients from various backgrounds.

Preferred Experience\n

• A minimum of 2 years of relevant clinical experience in mental health settings.

• Demonstrated skills in crisis management and in implementing evidence-based therapeutic interventions.

• Experience working with hospice patients and their families is highly desired.

Compensation and Benefits\n

The salary range for this position is competitive, between USD 70,000 and 85,000 annually, commensurate with experience.
