This full-time position offers the flexibility of remote work while being an integral part of our Hospice Center. In this remote position, you will connect with clients through virtual platforms as well as in-person interactions within the Hospice Center. Key Responsibilities\n

• Assess and treat clients dealing with major mental illnesses and severe emotional disorders through individual, group, and family therapy sessions.

• Create and implement personalized care plans, ensuring seamless access to the necessary resources and services for clients.
\n
• Provide brief, evidence-based interventions and facilitate referral processes when comprehensive assessments are required.
\n
• Conduct timely crisis interventions and support programs to ensure clients’ mental health remains stable during challenging times.
\n
• Regularly monitor client progress, ensuring that treatment remains focused on their presenting problems and goals.
\n
• Document all interactions and treatment plans promptly and accurately to aid in continuous improvement efforts.
\n
• Participate in safety assessments, identifying and mitigating potentially unsafe situations for both clients and staff members.
\n
• Engage in quality improvement initiatives, contributing to the refinement of agency service delivery and patient outcomes.

Patient Population / Client Focus\n

Your primary focus will be patients within the hospice setting, many of whom may be grappling with intense emotional and psychological challenges during their end-of-life journey. You will be instrumental in providing empathetic support, tailored therapeutic interventions, and a compassionate presence, fostering a supportive environment for both clients and their families.

Qualifications\n

• A Bachelor’s degree in Social Work is required; a Master’s degree is preferred.

• A valid and current Licensed Clinical Social Worker (LCSW) license in the state of Colorado.

• Demonstrated ability to build trust with clients, showcasing excellent interpersonal skills.

• Proficiency in crisis management, with a solid foundation in risk assessment methodologies.

• Strong organizational skills complemented by the ability to manage time effectively in a remote setting.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from USD 70,000 to 85,000, commensurate with experience.
