Key Responsibilities\n

• Conduct thorough assessments of clients through phone and in-person meetings to gather meaningful insights into their social and emotional circumstances.

Patient Population / Client Focus\n

Your primary clientele will include individuals and families affected by terminal illnesses, along with other mental health challenges in the hospice care setting. You will work with clients from various backgrounds and age groups, adapting your approach to meet their unique psychological and social needs. Your ability to build trusting relationships is critical in this role, as many clients rely on your support during some of their most vulnerable moments.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is essential.

• Current Licensed Clinical Social Worker (LCSW) designation in the state of Colorado is required.

Compensation and Benefits\n
