Work Environment\n

This role predominantly operates in a remote setting, allowing flexibility and work-life balance. The remote work environment supports our commitment to maintaining the health and safety of both our staff and the individuals we serve. However, for specific evaluations and team collaboration, opportunities may arise requiring visits to local facilities, such as our Hospice Center in Fort Collins, CO.

Key Responsibilities\n

The Licensed Clinical Social Worker will play a vital role in providing essential services to children and families engaged in our Early Intervention programs. Key responsibilities include:

• Conducting comprehensive evaluations and assessments aimed at identifying the developmental needs of infants and toddlers, ensuring informed decision-making regarding their care.

• Collaborating with team members to develop and implement Individualized Family Service Plans (IFSPs), actively monitoring and reporting on outcomes while maintaining alignment with each family's objectives.

• Building strong rapport with children, families, and interdisciplinary team members to create an atmosphere conducive to healthy learning and development.

• Equipping families with information and support mechanisms to enhance their child's developmental process, employing evidence-based practices tailored to individual learning styles.

• Utilizing coaching models and natural environment teaching methodologies to promote engagement during interventions.
\n
• Ensuring compliance with all federal and state regulations, upholding the highest quality of care standards.

Patient Population / Client Focus\n

Our focus is on supporting children aged birth to three years who may be experiencing delays or disabilities. As a clinical social worker within our Early Intervention Services, your expertise will be instrumental in guiding these young individuals towards developing essential physical, cognitive, social/emotional, and communication skills. Your work will directly contribute to improving outcomes for these children and their families.

Qualifications\n

Qualified candidates must meet the following requirements:

• A Master's Degree or Doctorate in Social Work.

• Current licensure as a Licensed Clinical Social Worker (LCSW) recognized by the Department of Public Health.

• A valid driver's license for potential travel between service locations.

• Demonstrated adaptability in addressing varied learning styles and the ability to develop individualized treatment plans addressing unique family dynamics.

Preferred Experience\n

While not required, having at least one year of direct experience working with children within early intervention settings is strongly preferred. Exposure to Natural Learning Environment Practices is also advantageous.
