Work Environment\n

This position is offered as a remote role, allowing for flexibility in both your personal and professional life while maintaining a strong connection to the community we serve. Regular communication through virtual collaboration tools and scheduled in-person visits will be essential for maintaining quality care. Our work hours are primarily Monday through Friday from 8:00 AM to 4:30 PM, though flexibility may be required for evening or weekend visits based on client needs. Occasional on-call responsibilities may also arise to address urgent client situations.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, considering their emotional, mental, and social circumstances.

• Develop personalized care plans in collaboration with clients and their families, ensuring that individual goals and preferences are prioritized.

• Provide one-on-one counseling and emotional support to assist clients in managing life transitions such as moving to assisted living facilities or coping with loss.
\n
• Advocate for clients' rights and connections to needed community resources, ensuring they receive timely access to services such as healthcare, housing, and social engagement opportunities.

• Work closely with a team of healthcare professionals, including nurses and caregivers, to coordinate comprehensive care and address various client needs effectively.
\n
• Monitor client well-being through routine visits, addressing changes in living conditions and needing to act on potential safety concerns.

• Facilitate effective communication among family members, fostering a positive support network for our senior clients.
\n
• Educate clients and families about available community resources, support groups, and programs that enhance social engagement and improve quality of life.

• Maintain meticulous documentation of client interactions, assessments, care plans, and progress notes using electronic health record (EHR) systems.

• Stay informed about evolving social work practices and policies to ensure compliance with ethical standards and best practices.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• A valid and current state licensure as a Licensed Master Social Worker (LMSW) is mandatory.

• Proficient understanding of social work principles, challenges of aging, and community resources available in Fort Collins and beyond.

• Familiarity with electronic health record (EHR) systems is desirable, along with general computer proficiency.

Compensation and Benefits\n
