Work Environment\n

This position will be conducted remotely, enabling you to connect with clients from the comfort of your own environment while providing essential mental health and addiction recovery support. As part of the hospice center team, you will collaborate closely with a multidisciplinary group of providers, including clinical directors and support staff, ensuring that our patients receive the highest quality of care tailored to their needs.

Key Responsibilities\n

• Conduct thorough assessments to identify the counseling needs of your clients, developing an individualized treatment plan that promotes recovery and rehabilitation.

• Utilize DSM criteria for substance abuse diagnosis and assess the necessary level of care based on ASAM criteria.
\n
• Prepare detailed substance abuse counseling plans as part of the client’s overall treatment strategy, ensuring alignment with established best practices.
\n
• Create a comprehensive discharge and continuity of care plan to facilitate a seamless transition to post-treatment support services.
\n
• Document all patient interactions, including assessments, treatment progress, referrals, and follow-ups, maintaining accurate clinical records in accordance with regulatory standards.

• Manage a caseload of clients, providing ongoing treatment planning and case management updates as necessary.
\n
• Coordinate support services involving family engagement and collaborate closely with external referral sources, such as legal representatives, schools, and employers.

• Address crisis-oriented issues using a multidisciplinary approach, facilitating effective interventions to ensure client safety and well-being.
