Key Responsibilities\n

• Conduct thorough psychosocial assessments for clients in hospice care, addressing their mental, emotional, and social needs.

• Develop and implement individualized care plans in collaboration with an interdisciplinary team, including physicians, nurses, and spiritual counselors.

• Facilitate family meetings to provide support, education, and guidance to loved ones during challenging times.
\n
• Maintain accurate documentation of assessments, care plans, and therapeutic interactions using our electronic health record system.

• Stay informed on the latest best practices in palliative and hospice care to enhance service delivery.
\n
• Actively participate in regular team meetings to discuss client progress, share insights, and support colleagues’ professional development.

Patient Population / Client Focus\n

In this role, you will work with a diverse population, primarily focusing on patients with life-limiting illnesses and their families. Our clients come from various backgrounds, each with unique cultural needs and values. You will play a critical role in ensuring that care is tailored to respect these values while addressing the emotional and psychological complexities associated with end-of-life stages.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited program.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in New Jersey.

• Strong foundational knowledge of hospice and palliative care principles.

• Excellent communication skills, both verbal and written, with a passion for supporting clients and families.

Compensation and Benefits\n
