Geriatric Nurse Practitioner: Primary Care & Telemedicine

PACE Southeast Michigan

Pace Southeast Michigan is seeking a Nurse Practitioner (NP) to provide primary care to a designated panel of participants. This role involves managing acute and chronic care under the supervision of a Physician‑In‑Charge, with responsibilities including chronic disease management, preventive care services, and documentation in medical records. The ideal candidate will have 3-5 years of RN experience in a clinical setting, preferably with geriatric expertise, and must maintain a current Michigan NP License.
