Located in Fairfield, CA, you'll find a rewarding career and a vibrant community to call home. *Primary Care Physician Role Overview:* The Primary Care Physician provides comprehensive primary care services to a panel of veterans, ensuring continuity of care within a multidisciplinary healthcare team. The provider will be responsible for the assessment and care of new and returning patients with any number of medical problems (e.g., hypertension, diabetes, ischemic heart disease, COPD, depression, chronic pain). The care may be delivered via face-to-face, telehealth, or video visits though this position may require occasional travel to VISN medical centers or Community-Based Outreach Clinics (CBOCs). Responsible for ordering diagnostics, consultations, medications, and other therapies as medically indicated, meeting preventative health maintenance needs, and completing all required VA computer documentation including clinical reminders and screenings in a timely manner. Telehealth and/or video visits may only be conducted from sites of care within the VA Northern California Health Care System or VA-approved alternate work sites.
