Lead Case Manager

Seedco

Job Description

Job Description

POSITION OVERVIEW:

Seedco is an affiliate of The Acacia Network, one of the leading human services organizations in New York City and one of the largest Hispanic-led nonprofits in the State, providing integrated, culturally competent, and trauma-informed programs in the areas of health, housing, social services, economic development, and cultural revitalization to more than 150,000 individuals annually. Seedco is a national nonprofit organization that advances economic opportunity for people, businesses and communities in need.

The Recovery Works program responds to the reality that fewer than one in five individuals exiting treatment maintain long-term employment. By pairing clinical recovery with training, employment linkages, and retention coaching, the program promotes sustainable economic independence. The Recovery Works program integrates workforce development and recovery support services to help individuals in substance use or behavioral health treatment achieve sustained employment and stability. Services are trauma-responsive, evidence-informed, and coordinated through strong partnerships with clinical providers, employers, and community organizations. Building on more than 25 years of Seedco’s workforce and family-strengthening experience, the program connects job readiness, sector-based training, and long-term coaching with recovery services. opportunities.

The Lead Case Manager provides trauma-responsive, person-centered case management to individuals in recovery who are transitioning into the community. The role partners with each participant to assess strengths, needs, goals, and barriers; develop and monitor an individualized service plan; and connect the participant with community-based resources that support recovery, stability, and long-term self-sufficiency. Key areas of support include employment, education and occupational training, housing, benefits, health and behavioral health care, transportation, legal services, and peer or recovery supports. The Lead Case Manager also promotes consistent service quality by coordinating complex cases, supporting case conferencing, and maintaining timely, accurate documentation and outcome data.

KEY ESSENTIAL FUNCTIONS:

· Complete comprehensive, strengths-based assessments addressing recovery, housing, employment, education, health, benefits, transportation, legal, family, and other stabilization needs.

Collaborate with each participant to develop, document, and regularly update an individualized service plan with measurable goals, action steps, responsible parties, and target dates.

· Provide ongoing case management, coaching, advocacy, and follow-up that support successful community transition, recovery stability, and participant self-sufficiency.

· Connect participants to appropriate community resources, including recovery supports, housing, public benefits, health and behavioral health care, transportation, legal assistance, childcare, and other essential services.

· Coordinate employment services, including career assessment, job-readiness support, résumé and interview preparation, referrals to employers, and connections to occupational training, education, credentialing, and apprenticeship opportunities.

· Maintain regular contact with participants and service partners; monitor progress toward plan goals; identify emerging barriers; and adjust interventions and referrals as needed.

· Lead case conferences for complex or high-need situations and coordinate services with treatment providers, peer specialists, workforce staff, employers, training providers, and community organizations.

· Apply motivational interviewing, trauma-responsive practices, harm-reduction principles, and culturally responsive approaches while maintaining professional boundaries and participant confidentiality.

· Respond appropriately to participant crises, safety concerns, relapse-related needs, and changes in level of care in accordance with organizational policies, mandated reporting requirements, and emergency protocols.

· Complete timely and accurate case notes, assessments, service plans, referrals, releases, progress updates, and outcome documentation in required databases and files.

· Support continuous quality improvement by reviewing documentation, tracking participant milestones, identifying service gaps, sharing resource information, and contributing to program reports and partner meetings.

REQUIREMENTS:

· Bachelor’s degree in social work, counseling, psychology, human services, criminal justice, public health, or a related field required. A relevant combination of education, professional experience, and recognized credentials may be considered in accordance with organizational policy.

· At least three years of progressively responsible case management experience serving adults with substance use, behavioral health, reentry, homelessness, or other complex service needs; experience coordinating complex cases is strongly preferred.

· Demonstrated knowledge of recovery-oriented systems of care, community reentry, workforce development, public benefits, housing resources, and community referral networks.

· Experience developing individualized service plans, documenting participant progress, coordinating multidisciplinary services, and using participant data to support outcomes.

· Strong engagement, assessment, crisis-response, advocacy, organization, written communication, and interpersonal skills.

· Ability to work effectively with people from varied backgrounds, maintain confidentiality and professional boundaries, and use a trauma-responsive, person-centered approach.

· Proficiency with Microsoft Office and electronic case-management or client-tracking systems; ability to maintain accurate records and meet documentation deadlines.

· Ability to travel locally to community partners and service locations as needed. Valid driver’s license and access to reliable transportation may be required based on program operations.
