Key Responsibilities\n

• Conduct detailed assessments and evaluations of clients to identify their mental health needs, social support systems, and treatment objectives.

• Provide empathetic one-on-one counseling sessions focusing on a range of mental health and psychosocial issues specific to hospice care.

• Facilitate engaging group therapy sessions aimed at addressing shared mental health concerns, fostering support networks, and delivering psychoeducational resources.

• Develop and implement tailored treatment plans, incorporating measurable goals and evidence-based interventions that align with the unique needs of each client.

• Deliver essential crisis intervention services, including thorough risk assessments, safety planning, and liaising with emergency resources.

• Advocate for clients, ensuring they have access to necessary services and assisting them in navigating systems of healthcare and community resources.

• Maintain meticulous and confidential client records, including evaluations, treatment plans, progress notes, and outcome measures in accordance with legal and ethical guidelines.

In this role, you will predominantly work with clients within a hospice setting, often facing end-of-life issues. Compensation and Benefits\n
