Key Responsibilities

As a Physical Therapist at Concentra Rehabilitation Center, you will:

• Conduct thorough assessments and evaluations of patients' physical conditions to develop effective treatment plans that align with physician directives.
• Implement a variety of evidence-based therapeutic interventions across multiple specialties, including orthopedic, pediatric, neurologic, and sports medicine.
• Utilize advanced therapeutic techniques and technologies to facilitate patient recovery in their homes, ensuring comfort and accessibility.
• Provide education and training to patients and caregivers to maximize engagement and adherence to rehabilitation protocols.
• Maintain accurate and timely documentation of patient progress and treatment outcomes, ensuring compliance with regulatory requirements.
• Meet minimum utilization goals while supporting organizational objectives to enhance service delivery and patient satisfaction.

Patient Population / Client Focus

This role serves a diverse array of patients, from those recovering from orthopedic surgeries to those needing specialized neurological rehabilitation. Your ability to form genuine connections with each patient is essential in helping them exceed their functional goals and regain independence.

Qualifications

To be successful in this position, you must have:

• A Bachelor's Degree in Physical Therapy, along with the necessary state licensure to practice as a Physical Therapist.
• A valid Doctorate degree in Physical Therapy (DPT) and completion of an APTA or CAPTE-accredited physical therapy program.
• Basic Life Support Certification from the American Heart Association.
• Excellent interpersonal and communication skills, both written and verbal.
• Proficiency in word processing and basic computer skills for documentation and scheduling.
• A desire to build and maintain strong relationships with patients and the community.

Preferred Experience

We are seeking candidates with at least two years of hands-on experience in physical therapy. Experience in home health settings and familiarity with evidence-based treatment practices will be highly advantageous.
