This position is structured to be performed remotely, providing significant flexibility. While the work is conducted from home, our Hospice Center is available for occasional in-person interactions and team building. Key Responsibilities\n

• Conduct thorough bio-psychosocial assessments to understand patient needs and eliminate barriers to treatment.

• Develop and implement individualized treatment plans that utilize evidence-based therapeutic modalities, both individually and in group settings.

• Document social work activities in patient medical records in accordance with departmental standards, maintaining clear and accurate records of care.
\n
• Collaborate closely with multidisciplinary teams, including medical and support staff, to ensure comprehensive patient care and encourage treatment adherence.

• Provide education to clients and families about mental health issues, resources, and the importance of following treatment recommendations to enhance patient outcomes.

• Advocate for patients’ needs within the community, focusing on self-determination and empowering clients to enhance their social support systems.

• Participate in ongoing professional development and training opportunities to maintain expertise and stay current with best practices in clinical social work.

Patient Population / Client Focus\n

The LCSW will work with a diverse population, tackling a range of psychosocial challenges related to mental and physical health issues. Our clients may include individuals facing severe emotional distress, complex family dynamics, and other factors impeding their psychosocial functioning. The therapist plays a crucial role in facilitating positive change, ensuring clients feel understood and represented throughout their therapeutic journey.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Must hold a valid Licensed Clinical Social Worker (LCSW) designation from the State Board of Social Work Licensure.
