Description

• Conduct patient evaluations in either inpatient or outpatient settings, carefully crafting personalized treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and functional abilities
• Collaborate closely with nursing staff, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
• Guide patients and their families through therapeutic goals, discharge plans, and at-home exercise regimens
• Document assessments, treatments, and patient progress following established facility and regulatory standards
• Engage in interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

Certifications

• Current licensure as a Physical Therapist in Minnesota
• CPR certification preferred
• Additional specialized certifications (neurological, geriatric) welcome but not mandatory

Skills

• Strongly preferred: previous relevant experience
