Primary Care Physician (MD/DO)

Join Our Team OnePlus Healthcare Group is seeking a compassionate, dedicated, and highly skilled Primary Care Physician (MD/DO) who is Board Certified or Board Eligible in Internal Medicine or Family Medicine to provide exceptional, patient-centered care to our growing community. We are looking for a physician who is passionate about preventative medicine, chronic disease management, and building lasting relationships with patients. Key Responsibilities Provide comprehensive primary care services, including preventive, acute, and chronic disease management. Perform complete patient evaluations, establish diagnoses, and develop individualized treatment plans. Promote health education, wellness, and preventive care to improve long‑term patient outcomes. Coordinate patient care with specialists, hospitals, ancillary providers, and interdisciplinary care teams. Maintain accurate, timely, and compliant medical documentation within the electronic medical record (EMR). Review laboratory and diagnostic results and communicate findings with patients in a timely manner. Participate in quality improvement initiatives, clinical meetings, and value‑based care programs, including HEDIS, Medicare Advantage, and Risk Adjustment initiatives. Ensure compliance with all federal, state, and organizational clinical standards and regulatory requirements. Foster strong patient relationships through compassionate communication and continuity of care.
