Description

American Traveler is seeking a Home Health Physical Therapist with 1+ years of recent experience and HCHB/OASIS proficiency to care for a geriatric caseload in the Mossyrock & Morton area.

Details

• Home Health setting serving a geriatric patient population
• Day shift, 5x8 hours, 8:00 AM-5:00 PM
• Average caseload of 5-7 patients per day
• On-call required approximately every 6th weekend
• Territory covers Mossyrock & Morton area, with flexibility required across the territory
• Uses HCHB (Homecare Homebase) EMR

Requirements

• Minimum 1 year of recent relevant Home Health experience required
• BLS certification required
• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in HCHB charting system required
• Active driver's license and personal transportation required
• License required if applicable
• Hospice/Home Health Checklist mandatory for consideration

Additional Information

• Responsibilities include home exercise program development and patient/family education
• Timely documentation and communication with the care team required
• Monday orientation with billable modules completed during this time
• Scrub attire required, any color, no jeans permitted
• iPad provided; mileage reimbursed at IRS standard rate
• Not open to first-time travelers
• Does not accept local candidates
• Travel pairs accepted, though each traveler is assigned an individual territory and must have their own transportation
• Former employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
• Requested time off (RTO) must be submitted at the time of application
