Licensed Social Worker Assistant Director Full‑time, Salary Range: $52,000 to $72,000, Location: CareOne at Somerset Valley. Position Overview Balance Life & Work with a New Career Opportunity. Job Description The Assistant Director of Social Services will manage daily coordination and oversight of the Social Service department, ensuring compliance with company policies, state and federal guidelines, and the National Association of Social Workers' Code of Ethics. Responsibilities include supervising staff, providing direct services, advocacy, counseling, service coordination, education, in‑servicing, policy and program development, and community relations. Essential Duties and Responsibilities Plan and direct a comprehensive social service program supporting residents and families. Participate in interdisciplinary team approach to serve the Center population. Provide therapeutic interventions helping residents cope with social and psychological aspects of illnesses, impairments, or disabilities. Maintain written documentation in the resident medical record per company policy. Perform initial evaluation/assessment of each resident’s psychosocial needs; provide ongoing documentation quarterly. Develop social service component of resident's plan of care, identifying specific problems, goals, and approaches. Participate in and coordinate interdisciplinary care planning conferences at admission and required intervals. Ensure that residents/families have input into the care planning process. Act as liaison between residents, families, outside agencies, and the Center Administrator to uphold resident rights. Conduct family meetings as needed (new admission group meetings, support group, family and friends council). Function in a management capacity, adhering to all policies and procedures of the Center. Perform administrative functions such as completing necessary forms and reports under the Administrator’s direction. Facilitate discharge planning and develop organized discharge plans for all residents. Ensure safety of all Center customers to minimize fire and accident risks. Ensure compliance with legal, safety, health, fire and sanitation codes, including fire and disaster plans and MSDS. Coordinate admissions, generate referrals, and maintain relationships with area referral sources. Verify insurance coverage through computer systems or verbal authorization. Conduct tours of the facility for outside interested parties. Position Requirements Master’s Degree in Social Work or a human services field. Possess a CSW or LSW within the state of operation. 3+ years of Social Work Experience. Long‑term care or skilled nursing experience preferred.
