Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of the Registered Nurse.
- Collect data on residents’ physical and psychosocial status; communicate findings to the RN and Interdisciplinary Team.
- Participate in developing an age-appropriate, culturally relevant plan of resident care and contribute to care plan revisions based on evaluation of outcomes.
- Collaborate with residents, families, and team members to ensure comprehensive care and safe, quality outcomes; coordinate referrals as needed.
- Maintain current nursing knowledge and competency; participate in ongoing education and inservice activities; support mentoring or orientation of staff as assigned.
- Demonstrate clinical leadership in the professional practice setting and uphold safety, infection control, and standard precautions.

Requirements
- Active LPN license in New York State
- BLS certification (AHA or RQI only)
- Minimum 1 year of experience serving in an LPN capacity in a LTC facility
- Ability to care for adult and geriatric residents and work in a long-term care setting
- Eligibility to work in the United States and to coverage within the stated shift and location
