Key Responsibilities\n

• Collaborate with physicians and other healthcare professionals to communicate patient progress, needs, and treatment concerns effectively.

• Maintain precise and timely documentation, including clinical assessments and progress notes, in accordance with the department’s policies and procedures.

• Work closely with your colleagues on treatment planning initiatives, participating in the development of personalized treatment and discharge plans based on thorough psychosocial evaluations.

• Provide counseling services that include conducting comprehensive assessments and implementing crisis interventions for patients facing emotional and behavioral issues.

• Assist clients with discharge planning, ensuring a seamless transition process by coordinating with various regional providers to facilitate access to essential services.

• Engage with patients' families and support networks to discuss treatment options, progress, and effective discharge strategies.
