Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of clients to evaluate their needs and strengths, collaborating with the treatment team to develop individualized care plans.

• Provide therapeutic interventions for individuals, families, and groups facing a variety of challenges, leveraging evidence-based practices to address mental health issues.

• Facilitate care coordination by liaising with healthcare providers and community organizations, ensuring that clients receive necessary resources such as housing assistance and trauma-informed support.

• Document treatment plans and client progress thoroughly, complying with professional standards and organizational policies, while ensuring confidentiality and security of patient information.

• Respond effectively to crisis situations, employing strong problem-solving skills to intervene in emergencies such as mental health crises and child welfare concerns.

• Conduct follow-ups with clients to monitor their progress and provide continued support, reassessing their needs as necessary.

Patient Population / Client Focus\n

This position focuses primarily on clients in hospice care, many of whom are navigating significant life changes related to chronic illness, end-of-life issues, and emotional distress associated with these conditions. You will have the unique opportunity to support patients and their families, helping them navigate grief, anxiety, and other psychosocial challenges during some of the most difficult periods of their lives.

Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited institution.

• A valid and active LCSW license in California, with a minimum of two years of clinical experience in social work.

Compensation and Benefits\n
