Work Environment

This position is strictly onsite, primarily situated within a school facility. Key Responsibilities

• Conduct thorough assessments by taking comprehensive health histories and performing physical exams, carefully documenting findings to inform future patient care.
• Diagnose and manage acute and chronic health conditions autonomously, strictly adhering to the Nurse Practitioner scope of practice as defined by New York State laws.
• Order, interpret, and follow up on laboratory tests, radiology studies, and other diagnostic procedures to guide treatment decisions.
• Develop and implement individualized treatment plans that include pharmacologic and non-pharmacologic interventions, while engaging patients and their families in the decision-making process.
• Establish measurable patient outcomes and goals, regularly evaluating the effectiveness of treatment plans and making adjustments as necessary.
• Enhance patient education through health promotion activities, anticipatory guidance, and wellness care initiatives, demonstrating a commitment to preventative care.

Patient Population / Client Focus

This role primarily focuses on the adolescent and young adult population within the school setting. The Nurse Practitioner will address a wide range of health concerns, including preventive care, chronic illness management, mental health issues, and acute health conditions. Qualifications

• A Master's Degree in Nursing as a Nurse Practitioner is required, reflecting advanced understanding and clinical competency.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses must be maintained.
