Housing Case Manager — Help Clients Find and Keep Housing

RADIAS Health

RADIAS Health seeks a Housing Case Manager to deliver housing support services in Hennepin County. The role blends community work with home-office tasks to help clients obtain and maintain housing while navigating benefits and paperwork. Requirements include a Bachelor's degree in Psychology or Social Work (or 4,000 hours supervised experience), a valid driver's license, and capacity to transport clients.
