Our organization is in search of a skilled and collaborative Nurse Practitioner (NP) to enhance our Interventional Cardiology program located in San Francisco. This role involves direct collaboration with Interventional Cardiologists to facilitate comprehensive care for patients facing complex cardiovascular and structural heart conditions. The position emphasizes close coordination and relationship-building with teams specializing in Advanced Heart Failure (AHF) and Cardiothoracic Surgery (CTS) .

Role Responsibilities

The Interventional Cardiology Nurse Practitioner is essential in managing patient care throughout various stages, including:

• Outpatient clinic visits
• Inpatient evaluations
• Procedural planning
• Post-intervention follow-up

The NP collaborates closely with Interventional Cardiologists to ensure effective clinical care delivery while maintaining seamless coordination with AHF and CTS services as necessary. Although invasive procedures are conducted by Interventional Cardiologists, the NP plays a vital role in preparing patients, providing education, and managing post-procedural care to guarantee optimal outcomes and an excellent patient experience.

Key Duties

• Clinical Care:
  • Deliver advanced clinical care to cardiac patients in both outpatient and inpatient environments, including initial evaluations, follow-up appointments, and hospital consultations.
  • Conduct thorough cardiovascular assessments, order and interpret diagnostic tests, and collaborate with Interventional Cardiologists to formulate treatment plans.
  • Assist with pre- and post-procedural care for interventional and structural heart procedures.
• Care Coordination & Multidisciplinary Collaboration:
  • Work alongside Interventional Cardiologists to support daily clinical operations and patient management.
  • Coordinate care with AHF and CTS teams to ensure timely and integrated multidisciplinary care.
  • Engage in multidisciplinary heart team and valve conferences, contributing to patient selection and care strategies.
  • Facilitate transitions of care, including hospital discharge planning and outpatient follow-up.
• Patient Education & Communication:
  • Inform patients and their families about cardiovascular conditions, recommended treatments, procedural expectations, and recovery processes.
  • Conduct both in-person and telehealth consultations, managing patient communications through the Epic electronic health record (EHR).
  • Act as a clinical resource for patient inquiries, symptom management, and follow-up coordination.
• Program Support & Quality Initiatives:
  • Contribute to quality improvement, patient safety, and patient experience initiatives within the Interventional Cardiology program.
  • Assist with clinical research activities, registry participation, and program development initiatives as applicable.

Patient Focus

The Nurse Practitioner will provide care for patients with intricate cardiovascular and structural heart diseases, including:

• Structural heart disease
• Aortic, Mitral, and Tricuspid valve disorders
• Advanced heart failure necessitating interventional therapies
• High-acuity cardiology patients managed by Interventional Cardiology

Qualifications

• Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP).
• Licensure & Certifications:
  • Current Nurse Practitioner license in California.
  • DEA Controlled Substance Registration Certificate (CSRC).
  • Basic Life Support (BLS) certification.
  • Advanced Cardiac Life Support (ACLS) certification.
  • Additional certifications as required by the department.
• Experience:
  • Prior experience in cardiology is essential.
  • Experience in interventional cardiology, structural heart disease, heart failure, or inpatient cardiology is strongly preferred.
  • New graduate Nurse Practitioners will not be considered.
  • Proven ability to care for complex cardiac patients and work effectively in a multidisciplinary clinical setting.
