POSITION SUMMARY:

The Homeless Outreach & Engagement Specialist position is responsible for Working with Native Americans who are experiencing homelessness and in need of supportive services. This position serves as the bridge to any services individuals may qualify for within NAC. This person works as part of an interdisciplinary team comprising other NAC departments, specifically PSH liaison, Behavioral Health, Emergency Shelter, and Youth Services. This position will work independently with minimum supervision and is knowledgeable about community services available to urban Native Americans.

RESPONSIBILITIES:

• Conduct street outreach in the community to locate, engage and build rapport with the target population. Meet with applicants in both an office setting as well as the community throughout the metro Phoenix area
• Provide accurate and comprehensive information about all NAC services
• Complete VI-SPDAT as needed, work with PSH liaison with data entry into a Coordinated Entry system for housing services
• Navigates identified client services and assists the client in accessing these services
• Responsible for coordinating client care with NAC services with current goals and any new goals or needs that arise. Re-engage clients if in need of additional services and coordinates such services
• Serves as patient advocate ensuring that services that the client is connected with are being received
• Follow policies and procedures for agency vehicle use and company cell phone
• Assist potential applicants with obtaining needed documents required for services
• Create a filing system to organize each person’s information throughout the process
• HMIS data entry
• Provide updates periodically to management and CEO on defined program goals
• Maintain outlook calendar
• Other duties as assigned.

EDUCATIONAL/WORK EXPERIENCE REQUIREMENT:

• Highschool Diploma/GED
• Bachelors degree preferred

FAIR LABOR STANDARDS ACT:

This position is considered to be Exempt for overtime pay provisions as provided by the Federal Fair Labor Standards Act (FLSA) and any applicable state laws. Non-Exempt employees are entitled to overtime pay for hours worked in excess of forty (40) hours per workweek.
