Our mission is to eliminate health disparities and improve community well-being through compassionate, high-quality care.SJCH is seeking a compassionate Geriatrician to provide comprehensive, patient-centered care to older adults. This role focuses on preventive care, chronic disease management, cognitive assessments, medication management, fall prevention, advance care planning, and coordination of care across multidisciplinary teams.Key ResponsibilitiesProvide comprehensive primary and preventive care for older adultsManage acute and chronic medical conditionsConduct geriatric, cognitive, and functional assessmentsEvaluate and manage dementia, depression, frailty, and age-related conditionsPerform medication reconciliation and address polypharmacy concernsCollaborate with specialists, behavioral health providers, social workers, pharmacists, and caregiversSupport advance care planning and goals-of-care discussionsDocument patient care accurately in the EHRParticipate in quality improvement and community outreach initiativesSupervise Advanced Practice Providers as assignedAdditional RequirementsExperience providing care within faith-based or congregational settings, including partnerships with faith communities and religious institutions.Ability to support health education, screenings, wellness monitoring, care coordination, and community-based outreach initiatives.Willingness and ability to conduct home visits and provide care outside of a traditional clinical setting.Demonstrated cultural competency in caring for diverse aging populations, with an understanding of health disparities, social determinants of health, and the needs of underserved elderly communities.QualificationsMD or DO degreeCompletion of an accredited Internal Medicine or Family Medicine ResidencyBoard Certified or Board Eligible in Internal Medicine or Family MedicineFellowship in Geriatric Medicine preferredActive California Medical License, DEA, and BLS CertificationExperience caring for geriatric populations preferredBenefitsLoan Repayment Program EligibilityMedical, Dental & Vision Coverage403(b) Retirement Plan with Employer MatchPaid CME Days and Annual CME AllowancePTO and 13 Paid HolidaysRelocation AssistanceH-1B and J-1 Visa Sponsorship AvailableWhy SJCH?Mission-driven FQHC serving underserved communitiesCollaborative, interdisciplinary care modelLeadership and professional development opportunitiesMeaningful impact on the health and well-being of older adultsEqual Opportunity StatementSt. John's Community Health is an equal opportunity employer.
