F-ACT Clinical Care Coordinator

Job Summary A Community Bridges Inc. (CBI) Clinical Coordinator provides clinical and administrative supervision, clinical oversight, training, and mentoring for Case Managers and Rehabilitation Specialists on an assigned team. They ensure clinical quality and productivity standards are met and exceeded by direct reports, and that assessments and treatment plans are developed and implemented for each patient on their team. Responsibilities Provide clinical and administrative supervision and oversight. Train and mentor case managers and rehabilitation specialists. Ensure compliance with clinical quality and productivity standards. Develop and implement assessments and treatment plans for patients. Conduct outreach and engagement activities with the substance use and mental illness population. Qualifications and Requirements Bachelor’s degree related to behavioral health, or equivalent combination of education, training, and experience. At least 1 year of experience working with the SMI population, across multiple settings and levels of care, including mental health and substance abuse. At least 1 year of experience in a behavioral health position providing outreach and engagement. Valid Arizona Driver’s License with a 39‑month clear MVR. Ability to pass a criminal background check for jail visitation/clearance. Completion of Crisis Prevention Institute (CPI) training.
