Client Care Coordinator

Job Summary The Client Care Coordinator at Community Bridges, Inc. (CBI) is a shelter or temporary lodging site–based position devoted to managing a housing caseload and coordinating care for individuals and families experiencing homelessness. The role involves coordinating and monitoring both RBHA and non‑RBHA community‑based services, connecting clients to primary care or PCMH options, and using housing search software to match clients with appropriate housing units. Responsibilities Coordinate care and support services to help clients end their current episode of homelessness. Track, monitor, and facilitate delivery of RBHA and non‑RBHA community services. Act as liaison between clients and existing or new PCMH/PCP services. Utilize housing search software systems to assist clients in locating housing units. Complete data entry into CBI’s established data system and produce ongoing reports. Document and track required interactions in accordance with the Arizona Administrative Code, ADHS/DBHS Provider Manual, CARF, and CBI policies and procedures. Collaborate with multidisciplinary team members including nurses, counselors, and clinical leads. Qualifications High school diploma or GED required; associate degree or higher in behavioral health preferred. Minimum six months of recovery from substance use or mental health disorders. Minimum six months of paid related work experience with exposure to behavioral and medical crisis situations. One to three years of full‑time health‑care related work experience preferred. Valid identification and Arizona Fingerprint Clearance Card. Completion of CBI Peer Certification within 90 days of hire. Benefits Generous paid time off (5 weeks per year).
