Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and financial needs.

• Develop and execute personalized care plans in collaboration with interdisciplinary teams, ensuring comprehensive patient support.

• Provide individual and group counseling to patients navigating adjustment disorders, grief, and coping strategies.

• Coordinate with community organizations to organize post-discharge services, facilitating ongoing care for patients and families.

• Offer emotional support and education about treatment options, resources, and entitlements available to patients and their loved ones.

• Lead crisis intervention sessions, providing timely therapeutic support to patients and family members in distress.

• Maintain accurate and timely documentation in accordance with both legal requirements and organizational policies.

• Engage in hospital committees, contributing your expertise to social work-related issues and initiatives.

Patient Population / Client Focus\n

Our primary patient population includes individuals receiving hospice care, facing chronic illness, and their families who require support in managing complex emotional and psychosocial challenges. You will work intimately with patients from diverse backgrounds, placing great emphasis on cultural competency and understanding the unique needs associated with end-of-life care.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) in New York.

Compensation and Benefits\n
