Key Responsibilities\n

• Deliver educational support to patients and their families regarding various treatment options, available resources, and self-care strategies.

• Engage in crisis intervention as necessary, providing immediate support to patients and families in distress.

• Generate and implement comprehensive discharge plans tailored to meet patients’ unique needs while connecting them with essential community resources.

• Participate actively in hospital committees, offering social work expertise to enhance overall patient care protocols.

• Maintain accurate and timely documentation, ensuring compliance with hospital policies, professional standards, and legal regulations.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.
