The successful candidate will work onsite in our state-of-the-art rehabilitation facility located in Charlottesville, Virginia. Key Responsibilities

• Direct and manage patient treatment plans focused on enhancing functional independence through exercises and manual therapy tailored to individual capabilities.
• Oversee and mentor Physical Therapy Assistants, ensuring they implement treatment plans accurately and effectively.
• Conduct thorough evaluations of patients using diagnostic assessments to gauge their muscle, nerve, joint, and functional abilities.
• Analyze patients' home environments to identify potential barriers to recovery and recommend modifications to promote rehabilitation.
• Administer prescribed physical therapy services, including manual and therapeutic exercise, traction, and the use of physical agents.
• Fit and adjust orthotic and prosthetic devices, collaborating with physicians to modify plans as needed based on patient response.
• Document progress by preparing clinical records, evaluations, care plans, and discharge summaries, ensuring accuracy and compliance with state regulations.
• Engage as a member of an interdisciplinary team during case conferences to strategize treatment approaches for diverse client needs.
• Educate clients and their families on rehabilitation practices and the utilization of therapeutic equipment to promote effective home care.
• Maintain communication with case managers and physicians regarding patient progress and necessary adjustments to care plans.
• Complete clinical documentation and orders in a timely manner, adhering to branch policies for operational efficiency.

Patient Population / Client Focus

Qualifications

• Bachelor's degree from an accredited physical therapy program.
• A minimum of two years of experience working as a physical therapist, ideally in a rehabilitation or home care setting.
• Current and valid physical therapy licensure in Virginia.
• Current certification in CPR.
• Valid driver's license and proof of automobile insurance.
• Exceptional interpersonal skills, with the ability to engage effectively with patients, families, and multidisciplinary teams.

Compensation and Benefits
