Revive Active Action Therapy, Inc. is seeking a dedicated and skilled Full-Time Physical Therapist (PT) to join our team, focused on delivering exemplary rehabilitation services within a home care setting. As part of your role, you will be responsible for providing comprehensive therapeutic care to our clients, who may be sick, injured, or disabled, while also managing a range of important clinical operations. This position offers an annual salary ranging from USD 120,000 to 130,000.

Key Responsibilities

• Create a professional and collaborative atmosphere that fosters teamwork, patient safety, and exceptional clinical outcomes, encouraging full participation from all team members.
• Assess clinical improvement opportunities in collaboration with the Director of Therapy Operations (DTO) to enhance service delivery and patient outcomes.
• Develop and nurture relationships with employers, referral sources, and payer groups through regular communication and engagement activities, ensuring a steady flow of referrals.
• Utilize clinical systems proficiently to provide accurate documentation, optimal coding, and effective charge capture, while ensuring follow-through on all aspects of patient care orders.
• Maintain compliance with the Florida State Practice Act, relevant regulations, and Workers' Compensation (WC) guidelines for physical therapy.
• Drive metrics associated with patient and client experience and satisfaction, continuously seeking feedback for improvement.
• Adhere to the Physical Therapy Guidebook and ensure all treatment visits meet the established standards for quality care.
• Manage clinical outcomes systematically, discussing outlier cases with staff therapists to devise appropriate strategies for improvement.
• Optimize workflows related to patient referrals, scheduling, and overall patient management to enhance service delivery.
• Execute tailored therapy plans that prioritize early intervention and same-day evaluations for clients, thereby maximizing their recovery potential.

Patient Population / Client Focus

Your role will involve working closely with a diverse range of clients, including those recovering from surgeries, strokes, sports injuries, and chronic conditions. Your ability to customize therapy based on each client's unique needs and home environment is critical to achieving positive outcomes. You will be expected to build rapport quickly and communicate effectively with both clients and their families, ensuring they feel supported throughout their rehabilitation journey.

Qualifications

• Active Florida State licensure as a Physical Therapist.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Exceptional problem-solving skills, with the ability to analyze key facts, explore alternative solutions, and implement effective plans.
• Prowess in delivering outstanding customer service, coupled with a professional demeanor that instills confidence and trust in clients.

Preferred Experience

• Experience in home health care or related rehabilitation settings.
• Ability to work with electronic health record systems and familiarity with various therapy-related software.
• Knowledge of current physical therapy techniques and trends.
