Service Coordinator

About The Position We are seeking a compassionate, organized, and self‑motivated Service Coordinator to support residents by connecting them with community resources, programs, and services that promote independence, wellness, and quality of life. This position works closely with residents, community agencies, and property management to help residents successfully age in place and access the support they need. This position is employed and supervised by National Church Residences . The Service Coordinator will report directly through National Church Residences while working onsite at our property. Responsibilities Build relationships with residents and assess individual needs. Connect residents with community resources and supportive services, including: Transportation Financial assistance Housekeeping services Counseling and wellness programs Healthcare resources Meal assistance programs Coordinate educational workshops, health screenings, and wellness events. Develop and maintain partnerships with local agencies and service providers. Maintain a resource directory of community services available to residents. Encourage resident participation in onsite and offsite programs. Assist residents in developing support networks with family, friends, and community organizations. Monitor referrals and follow up to ensure services are meeting resident needs. Complete required documentation, care plans, and reporting in accordance with HUD and National Church Residences guidelines. Respond to resident concerns in a professional, respectful, and timely manner. Maintain confidentiality and professional boundaries at all times. Qualifications Preferred Education Bachelor's Degree in Social Work, Human Services, Psychology, Gerontology, or a related field. Experience Minimum of 2 years of experience in social services, case management, resident services, community outreach, or a related field. Experience working with seniors, families, or individuals with disabilities is preferred. Skills Excellent communication and interpersonal skills. Strong organizational and problem‑solving abilities. Ability to work independently and manage multiple priorities. Proficiency with Microsoft Office and basic computer applications. Ability to build partnerships and advocate for residents. Physical Requirements Ability to walk throughout the property and community. Ability to occasionally lift up to 25 pounds. Ability to work both indoors and outdoors as needed.
