Essential Duties and Responsibilities: Develop, implement, and adjust a conditioning/rehabilitation plan of care in partnership with patients, representatives, caregivers, and physicians Test and screen patients' physical strength to assist physicians in evaluating their level of function Educate and instruct patients, family members, or representatives about exercise programs and therapeutic procedures Accurately document observations, interventions, and evaluations pertains to patient care management and services provided Provide direct patient care as prescribed, including evaluation and treatment planning Review physicians' referrals and patients' medical records to determine diagnosis and physical therapy treatment required Minimum Qualifications (Knowledge, Skills, and Abilities): Graduation from an approved Physical Therapy curriculum Current and valid Connecticut Physical Therapist license Recent work experience in home health (preferred) Driver's License, Car Insurance, and Car Registration Strong clinical assessment, organizational, and communication skills Ability to handle multiple assignments simultaneously Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation Bilingual (Spanish) preferred Strong interpersonal skills and good judgment Ability to work well under pressure and stay calm and focused in emergent situations Be highly self-motivated and results-driven A desire for further education to keep up with the ever-changing medical advances Physical Demands and Work Environment: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. $90,000 - $120,000 ANNUALLY
