We are looking for providers who are experienced in interventional cardiology, must have a few years working in the field to be considered, new grads will not be considered going forward

CREDENTIALING

• Current CV (must be uploaded to bid)
• License: Active CA License
• DEA Required: Yes
• Boards: BC or BE within 3-5 years of residency
• Skills/Certs: ACTIVE FLURO is required

Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation.

o All HBS specialties require 2 years of current Hospital clinical activity in the specialty.
o Gaps greater than 3 months must be explained.
o Include current number of case logs upon presentation for surgical specialties.

ADDITIONAL DETAILS

Please ensure you review the practice description for all required procedures. It will be 75% Interventional and 25% noninvasive.

NUC reading/NUC activity will be required.
