Pay: $42/hour on W2 for Locals | $54/hour for Travelers Position Overview

Modoc Medical Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) with skilled nursing experience to join our team on a 13-week contract assignment.

The LVN will work under the direction of the Director of Nursing (DON) and Charge RN while collaborating with physicians, nursing staff, residents, families, and other departments to provide safe, high-quality, individualized care to residents in our skilled nursing facility.

This is an excellent opportunity for an experienced LVN who is committed to providing professional, compassionate, and resident-centered care.

• Perform comprehensive and individualized patient/resident assessments and identify changes in condition.

• Report abnormal findings promptly to the Charge Nurse and physician.

• Develop and implement individualized plans of care in collaboration with the healthcare team.

• Administer medications safely and accurately according to physician orders and facility policies.

• Perform physician-ordered treatments and nursing procedures in a timely manner.

• Perform clinical duties such as EKGs, blood draws, IV starts, wound care, and specimen collection , as qualified.

• Monitor and document patient responses to medications, treatments, and nursing interventions.

• Respond appropriately to emergency situations and initiate appropriate CPR/resuscitation protocols.

• Provide patient and family education regarding medications, treatments, care plans, and health needs.

• Maintain accurate and timely clinical documentation and patient records.

• Communicate patient conditions and changes effectively during shift reports and walking rounds.

• Follow all infection-control, safety, medication administration, and narcotics-control procedures.

• Assist with patient transfers and coordination of diagnostic tests and procedures.

• Support CNAs and other nursing staff to ensure assigned patient care is completed and documented.

• Maintain a clean, organized, and safe patient-care environment.

• Participate in staff meetings, orientation, continuing education, CPR training, and required in-service programs.

• Maintain patient confidentiality and protect the dignity, rights, and privacy of residents and families.

• Perform other duties as assigned by nursing management.

Required:

• Current and active California LVN license

• Current BLS/BCLS certification from AHA

• Minimum 1 year of nursing experience

• Skilled Nursing Facility (SNF) experience required

• Strong clinical assessment and critical-thinking skills

• Ability to respond effectively to emergencies

• Excellent verbal and written communication skills

• Ability to work collaboratively as part of an interdisciplinary healthcare team

• Strong organizational and time-management skills

• Dependable attendance and professional work ethic

• Ability to work independently while following the established chain of command

• Ability to safely perform the physical requirements of patient care, including walking, bending, lifting, and other related activities

The LVN must be comfortable providing care to residents who may be confused or exhibit challenging behaviors, including verbal or physical aggression. Candidates should demonstrate patience, professionalism, adaptability, and a commitment to treating every resident with dignity and respect.

• Caribbean Blue scrubs are required for all nursing staff.

• Must maintain current BLS/CPR certification throughout the assignment.

• Must comply with facility policies, regulatory requirements, infection-control standards, and professional nursing practices.

• Must report to work on time and maintain reliable attendance.

• Holiday coverage is not required for this assignment.
