Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments tailored to individual clients, focusing on their mental health, substance abuse issues, and medical needs within the context of primary care.

• Provide direct behavioral health services, including individual and group counseling, crisis intervention, and treatment planning to address immediate and long-term recovery goals.

• Document client progress, treatment plans, and clinical interventions electronically, ensuring compliance with health care regulations and quality improvement standards.

• Act as a liaison to connect clients with relevant community resources and support services, such as medication assistance programs, ensuring they have access to comprehensive support.

• Collaborate with Quality Improvement teams to develop and implement strategies for effective data collection and service evaluation.

Patient Population / Client Focus\n

You will work primarily with adults experiencing acute and chronic mental health issues, substance use disorders, and those in need of detoxification support. Your role will involve engaging with diverse populations, requiring an understanding of cultural sensitivities and the ability to provide age-appropriate interventions that respect the individuality of each client.

Qualifications\n

• Valid LCSW license in the state of California is required.

• A Master’s degree in Social Work (MSW) with at least three years of post-licensure experience in a clinical setting.

Compensation and Benefits\n
