About the Position

Key Responsibilities

• Perform and document comprehensive patient assessments, incorporating culture, age, developmental level, risk factors, and presenting complaint in accordance with RN scope of practice and applicable policies and procedures.
• Develop and prioritize individualized problem lists and care plans in coordination with the patient, family, and inter-professional team; determine desired outcomes aligned with the medical plan of care.
• Implement patient care therapies and procedures with clinical autonomy within scope of practice; explain procedures, treatments, and processes to patients and families and initiate actions to alleviate stress and discomfort.
• Recognize, respond to, and communicate changes in patient condition; conduct and document ongoing assessments and responses to procedures and therapies using the nursing process.
• Delegate tasks appropriately and monitor accuracy and completeness of delegated patient care activities performed by other team members.
• Facilitate patient discharge processes, including timely execution of discharge orders, discharge teaching, and coordination with Case Management; actively participate in unit-based quality improvement activities such as patient satisfaction, fall prevention, pressure ulcer prevention, and CAUTI reduction.

• Active, unrestricted Registered Nurse (RN) license in the state of Kentucky.
• Basic Life Support (BLS) certification required.
• Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN).
• Ability to communicate and collaborate effectively with all members of the care team.
• Proficiency with or ability to be trained on applicable computer systems and applications.
• Ability to assess age-specific patient safety issues and implement appropriate safety measures.

• Previous nursing experience in a clinical setting.
• Bachelor of Science in Nursing (BSN).
