Social Services Director – Sylvania Health Care Center Now Under New Management – Join Our Growing Leadership Team! As we continue building a culture centered on compassion, teamwork, accountability, and exceptional resident care, we are looking for talented professionals who want to be part of our growth. We are currently seeking an experienced and compassionate Social Services Director to join our leadership team. The Social Services Director plays a critical role in advocating for our residents and supporting their emotional, psychosocial, and social needs throughout their stay. This position works closely with residents, families, nursing, therapy, admissions, and the interdisciplinary team to ensure each resident receives individualized, resident-centered support. What You'll Do: Lead and oversee the facility's Social Services department Assess and address residents' psychosocial, emotional, and social needs Serve as an advocate for residents and their families Participate in care planning and interdisciplinary team meetings Assist residents and families with adjustment to long-term care and short-term rehabilitation Coordinate discharge planning and help ensure safe, successful transitions of care Provide support and resources to residents and families throughout their stay Maintain accurate and timely documentation in accordance with facility, state, and federal requirements Develop strong relationships with residents, families, community partners, and referral resources Collaborate with facility leadership to promote resident satisfaction and an exceptional overall experience What We're Looking For: Strong communication, organization, and relationship-building skills Compassionate and resident-focused approach Ability to navigate challenging conversations with professionalism and empathy Strong understanding of the needs of residents in a skilled nursing and long-term care environment Leadership skills and the ability to collaborate effectively across departments Previous Social Services experience in healthcare, skilled nursing, or long-term care preferred Education and credentials consistent with applicable state and federal requirements At Sylvania Health Care Center, successful team members are grounded in work ethic, integrity, and competence. #J-18808-Ljbffr Reliant Care Management Company
