Key Responsibilities\n

• Conduct thorough, ongoing assessments to ensure that the needs of patients are meticulously met.

• Communicate effectively with supervisors, direct care staff, family members, guardians, and other key stakeholders regarding patient progress.

• Outline a tentative schedule for appointments, home visits, and other essential observations to monitor work advancement.
\n
• Coordinate and facilitate home visits, ensuring compliance with the necessary in-home observations and chart reviews.
\n
• Implement and monitor the effectiveness of program plans, making adjustments where necessary to optimize care.
\n
• Participate actively in interdisciplinary team meetings and any relevant consultations for the seamless provision of care.
\n
• Document detailed monthly and quarterly progress notes to reflect patient development accurately.
\n
• Organize and conduct training sessions to ensure team members are proficient in implementing program plans and maintaining documentation standards.
\n
• Provide ongoing follow-up interviews, home visits, and observations to continue supporting the individual needs of clients.
\n
• Maintain meticulous records that are easily accessible to pertinent stakeholders, enhancing transparency and communication.

• Act as a liaison between our organization and the public mental health community, assisting in strategic goal development.

• Collaborate to create and refine training materials, enhancing program delivery for individuals.

Patient Population / Client Focus\n

In your role as a Licensed Professional Counselor, you will work with a diverse population that includes both adults and youth facing various challenges such as trauma, substance misuse, and family dynamics. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Oregon.

• Specialization in family therapy, couples therapy, substance abuse challenges, or trauma is highly desirable.
