Key Responsibilities\n

As a Licensed Professional Counselor, your day-to-day activities will include:

• Conducting initial and ongoing assessments to understand patient needs, tailoring treatment plans accordingly.

• Facilitating clear and open communication between supervisors, fellow team members, patients' families, and other involved parties to monitor individual progress continuously.
\n
• Scheduling appointments, home visits, and assessments, and overseeing your calendar to ensure all commitments are met.
\n
• Coordinating home visits for in-home observations, including engaging in thorough chart reviews and data analysis to track progress effectively.
\n
• Implementing and tracking the effectiveness of individualized treatment plans, making adjustments as necessary.
\n
• Participating actively in Interdisciplinary Team Meetings to contribute to collaborative treatment strategies and progress tracking.
\n
• Maintaining comprehensive records and documentation to ensure clarity and availability of information for stakeholders.
\n
• Providing ongoing support through follow-up home visits and interviews, ensuring clients feel supported throughout their journey.
\n
• Serving as a liaison between the public, the mental health community, and families, thereby fostering a supportive network that addresses developmental goals.

• Organizing training sessions to ensure team competence in delivering effective care and documentation processes.

Patient Population / Client Focus\n

You will service a diverse clientele, which includes both adults and youth, addressing a variety of mental health challenges such as anxiety, depression, trauma, and substance misuse. Your expertise will be essential in creating supportive treatment plans that accommodate the specific needs and contexts of each patient, ultimately enhancing their quality of life.

Qualifications\n

The qualified candidate will possess the following:

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.

• A minimum of two years of counseling experience in a clinical context.

• Strong knowledge of legal and ethical standards relevant to mental health practice.

• Exceptional interpersonal skills with the ability to build rapport with clients from various backgrounds.

Compensation and Benefits\n
