Key Responsibilities\n

• Conduct thorough psychosocial assessments to uncover the emotional, social, and financial challenges affecting patients during their healthcare journey.

• Collaborate with medical and clinical teams to create individualized care plans that address both clinical and psychosocial aspects of treatment.

• Provide individual and group counseling, utilizing evidence-based practices to assist patients in coping with anxiety, depression, grief, and adjustment issues.

• Facilitate connections with community organizations for post-discharge services, ensuring a holistic approach to continued care.

• Educate patients and families about treatment options, available resources, and strategies for navigating their healthcare experience.

• Implement crisis intervention techniques as necessary, providing patients with immediate emotional support and guidance during critical moments.

• Maintain meticulous documentation in accordance with legal standards and organizational policies to support continuity of care and improve patient outcomes.

• Actively participate in clinical committees and meetings, contributing insights on social work best practices and advocating for patient needs.

Patient Population / Client Focus\n

This position primarily focuses on serving the Latinx community, although any patient from diverse backgrounds is welcome. You will engage with individuals facing various challenges, including mental health disorders, chronic illness, adjustment to treatment, and socio-economic barriers. Your unique cultural understanding will contribute significantly to establishing trust and rapport with patients, ensuring better engagement and compliance with their care plans.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
