Key Responsibilities\n

• Conduct comprehensive client assessments to identify social, emotional, and environmental challenges that impact their well-being.

• Develop tailored service plans, inclusive of clear goals, therapeutic interventions, and mechanisms for tracking progress over time.

• Provide individual and family counseling, utilizing various therapeutic modalities to assist clients in navigating their difficulties.

• Engage in crisis intervention, performing risk assessments and safety planning for clients in urgent situations.

• Collaborate closely with clients to connect them to community resources that can support their recovery and wellbeing.

• Participate in case conferences and multidisciplinary team meetings to discuss client progress and align on care approaches.
\n
• Lead community outreach initiatives aimed at raising awareness of available services and advocating for client needs.
\n
• Document client interactions and maintain accurate case records in compliance with organizational and legal standards.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals undergoing detoxification and experiencing substance use disorders, along with co-occurring mental health conditions. By addressing both the psychological and social dimensions of care, you will empower clients to achieve meaningful recovery outcomes. Your advocacy will help dismantle barriers to accessing necessary resources and services, enabling clients to rebuild their lives.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in New York.

• Strong understanding of trauma-informed care and its application in treatment practices.

Compensation and Benefits\n
