Job Description

We are seeking a skilled Home Health Registered Nurse to provide care in the patient's home setting. The ideal candidate will have recent and relevant experience in home health care, with proficiency in using EPIC and OASIS for patient management.

Responsibilities

• Conducting start of care assessments and revisits for patients.
• Administering medication and managing patient care plans.
• Collaborating with interdisciplinary teams to deliver high-quality care.

Required Experience / Certifications / Licensure

• Recent and relevant home health experience.
• EPIC software proficiency.
• OASIS certification required.
• Active RN license.
