Position Summary Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person we serve’s abilities to function in the community. They assess, monitor, and follow up with the individuals we serve to ensure they meet basic needs and move toward recovery. They are responsible for all clinical documentation of consumer interactions. As a Clinical Case Manager, you will provide transportation to the individuals we serve in your vehicle (additional cost for insurance is reimbursed). Job Duties & Functions Help people we serve meet basic needs for food, clothing, shelter, personal safety, and general medical and dental care, and assist them with applications for income, medical, housing, or other benefits to which they are entitled. Identify those who require a protective payee and arrange this assistance. Ensure people we serve have access to needed services and community resources by arranging for transportation, or if necessary, by transporting them to the service location in personal vehicle. Provide comprehensive psychosocial services that include a continuum of high and low intensity services designed to improve or maintain people we serve’s abilities to function in normal social roles. These will include, but are not limited to, services such as: Help people evaluate strengths and symptoms, facilitate goal setting, and plan for appropriate services; Guide and instruct people in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, and use of transportation; Help people develop social skills, interests, and leisure time activities to provide a sense of participation and personal worth, including culturally appropriate daytime and evening activities; Help people find and use appropriate employment opportunities, vocational rehabilitation services, or supported work environments where appropriate; Assist people with special living arrangements in locating rehabilitative or supportive housing. Choices should allow each consumer to live with the appropriate level of support while encouraging increased responsibility. Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others to maximize benefits and minimize problems associated with their presence in the community. Identify and work with potential natural support systems such as neighborhood networks, churches, and community organizations. Fully utilize the consumer’s support systems to encourage treatment program engagement. Reach out to program‑eligible individuals, inform them of and educate them about available services at WellPower and in the community. Maintain a trauma‑informed environment of wellbeing. Perform other duties as assigned. Requirements & Qualifications Education Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or another related human services degree. Experience One year of experience in mental health or human services preferred. Must have a valid Colorado driver’s license and a recent clean driving record. Ability to transport the individuals we serve in your vehicle. Ability to obtain and carry Business‑Use coverage on your auto policy (additional cost is reimbursed). Shift / Hours This is a Monday‑Friday opportunity observing standard business hours. Physical Requirements Ability to sit, stand, bend and reach, drive a vehicle, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs.
