Occupational Therapist Home Health - Per Diem - Sign On Bonus - Full Time Available

Responsibilities/Essential Functions

• Provide professional occupational therapy services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the healthcare team

• Conduct home safety checks and environmental assessments of patient homes

• Develop and implement individualized plans of care to promote independence and physical and/or mental functioning

• Re-evaluate patients' ADL and iADL abilities and assist physicians in evaluating level of function

• Collaborate with other disciplines to ensure comprehensive care

• Effectively communicate with patients, families, and other healthcare professionals

• Ensure accurate, complete, and timely documentation of all patient interactions and services

Requirements

• Graduate of an OT education program accredited by the Accreditation Council for OT Education (ACOTE) or successor organizations

• Currently licensed to practice in Michigan

• Minimum of two (2) years of experience as an OT, preferably in a home care setting

• Ability to communicate effectively in English

• Able to access patient homes that may not be handicap accessible

• Ability to perform physical demands of the job, including walking, standing, sitting, reaching, lifting, and bending

Work Environment

• Work indoors, performing patient care assessments and tasks in patient homes

• Travel to patient residences and/or meetings

• Possible exposure to communicable diseases

Benefits

• Competitive pay rate of $55 - $65 daily
