Position SummaryProvide interpretation and translation services, culturally appropriate health education and information, informal counseling and guidance on health behaviors, and advocate for individual and community health needs. This position includes Client Orientation to Comprehensive Peri-natal Services Program (CPSP) initial assessment, trimester reassessments, postpartum assessment, interventions, and follow-up services in: Obstetrics, Nutrition, Health Education, Psychosocial and Individual Case Coordination.ScheduleFlexible, may require some evening and weekendsPrimary Duties and ResponsibilitiesOrient the patient to comprehensive perinatal care at entry of care and throughout the pregnancy as neededComplete assessments in the four components – obstetric, nutrition, health education and psychosocial usingapproved assessment formsComplete the Individualized Care Plan (ICP) with the patient following the initial assessments. Prioritize problemsand actions planned to resolve themProvide individual Perinatal Education, counseling, referrals and instructions as appropriate to the needs and risksidentified on the ICPReassess in each trimester to identify changes or new developments since the previous assessment and to providecontinued support for the patient’s strengths. Revise and update the ICP as neededPostpartum Assessment and Care PlanAssess the mother and infant postpartum and update the ICP. Provide appropriate interventions for the patientand her babyTrack patient’s attendance at appointments, and identify the reason for any missed appointmentsResponsible for issuance of Presumptive Eligibility if patient is qualified for the Program.Maintains OB/EDC log, Newborn log and sends monthly report to Managed Care, QA Director and Clinic Supervisor.Ensure that appropriate copies of the prenatal record are at the hospital during the Intrapartum period (34 weeksgestational age)Assist the patient to obtain Medi-Cal and explain what documents she must have to apply for Medi-Cal.Inform Supervisor of any issues or problemsTime StudiesMaintain a positive working relationship with staffOther duties as designated by the Other duties as designated by the Clinic Manager, Clinic Director, leading MedicalProvider, or Clinical Service DirectorMinimum RequirementsMicrosoft Office (Word, Excel, Outlook)Excellent Communication SkillsExcellent Customer ServiceBilingual English/Spanish preferredAbility to work as a team player and work independentlyReliable transportationMust be able to travel from facility to facilityThis position requires 10 % travel outside the local area.Required Education/Experience and/or Licensure/CertificationsMedical Assistant Certification or higherHigh School Diploma and/or GED equivalent required.Certified Comprehensive Perinatal Health WorkerAt least one year of full-time paid clinical perinatal experience and must be certified CPHWParticipate in ongoing trainings and continued education for skills developmentCommitment to goals and philosophy of Northeast Community ClinicCurrent CPR and/or BLS CertificationValid State IdentificationPhysical Requirements and Working ConditionsOSHA Category 2 – Involves no regular exposure to blood, body fluids, or tissues, but may require unplanned tasks that involve exposure to blood, body fluids, or tissues.The work is majority of the time non-sedentary in nature.The employee is regularly required to communicate, frequently required to use repetitive motions, move, remain stationary, regularly push, pull and lift up to 20 pounds and occasionally push, pull and lift up to 40 pounds.Must possess mobility to work in a standard office/clinic setting (in some cases both settings) and to use standard office/clinic equipment, including a computer, stamina to maintain attention to detail despite interruptions, read printed materials and use a computer, and communicate in person and over the telephone.Work Location: In person #J-18808-Ljbffr Northeast-Community-Clinic-4
