Job Opportunity

Position Overview

This position is available in POD6, covering the Pinal County area, which includes Maricopa, Casa Grande, Eloy, and Arizona City.

Key Responsibilities

Provide high-quality home care to patients in the specified coverage area.
Conduct assessments and create care plans based on patient needs.
Utilize OASIS knowledge to enhance patient care and documentation.
Administer wound care and manage related treatment plans.
Collaborate with healthcare teams to ensure comprehensive patient care.

Qualifications

Prior experience in home care is preferred.
Knowledge of OASIS guidelines is beneficial.
Experience in wound care is required.
Background in ICU, Emergency Department (ED), or Medical-Surgical (Med Surg) settings is a plus.
Strong communication skills and ability to work independently.
